Please use this form to submit your case. It must be filled out in its entirety.

    Use this section to submit your information and pictures.

  • I need your mother’s first name (NOT her maiden name), just her first name. Your mother won’t be included in the ritual. I need this information because she is your direct connection to the universe. If you were adopted please include the first name of your biological mother. If you do not have the name just write n/a.

    If you have ordered a love spell, such as a Return Lost Lover Spell, a Spell to Make someone fall in love with you, or any spell that involves your loved one, then please use this section to submit your loved one's information.

  • Information to a Third Party

    If your spell involves a third party, such as a break-up spell or a friendship spell, then use this section to submit the person's information. Enter all that you know. If you do not know all the details just leave them empty. If your case involves additional third parties then please email their information to me. If your case does not include any third parties just ignore this section.
  • This is only needed for spells that include another person, such as break-up spells. If you do not know the name of a third party, please leave it empty.
  • Let me know who this person is and which role she plays in the spell. Such as: The other person for the break-up spell, a friend for a friendship spell, etc.
